Basic information on health insurance for TUL students and doctoral students.

The principles of health insurance are regulated by the provisions of the Act of 27 August 2004 on financing health care services from public funds (Journal of Laws No. 164, item 1027, as amended). Students are subject to compulsory health insurance and have the right to use free medical care services in facilities that have concluded an agreement for the provision of healthcare services with the National Health Fund.


Students and doctoral students of Lodz University of Technology who are citizens of the Republic of Poland
  • Students and doctoral students of Lodz University of Technology who are 26 years of age and who are not subject to the obligation to be insured under any other title (e.g.: profession, business activity, disability pension, status of family member declared for the main insured person's insurance) for health insurance - upon their written request – their applications for insurance are filed by the university which pays for them the health insurance premium.

The declaration and the relevant ZUS form should be submitted to the Student Affairs Section,

ZZA- the application form for the student health insurance

ZCNA- the application form for the health insurance of a family member

ZIUA- the form of the notification of personal data change

The University takes out insurance from the date of submission of the above documents.

The fact of taking up employment or obtaining the right to insurance under another title must be reported to the university within 7 days (ZUS ZWUA form or declaration).

A student is required to declare family members for health insurance, provided they do not have any other title to health insurance. During this period, the University pays a health insurance premium. The student's right to healthcare services expires 4 months after graduation or removal from the list of students.


  • Students and doctoral students of Lodz University of Technology who are under 26 years of age benefit from their parents' health insurance – as family members.

In case of students/doctoral students residing in Poland, whose parents live, work and are subject to health insurance abroad, it is possible to issue a portable document S1/E109 by a foreign health insurance institution, which entitles the student/doctoral student to benefit from health care services in full scope on the territory of Poland. The above mentioned document should be submitted for registration in the National Health Fund Local Branch appropriate for the place of residence in Poland.

Registration of the portable document S1/E109 in the National Health Fund is possible if the student/doctoral student does not have his own title to insurance in Poland (e.g. employment, business activity, disability pension, registration in the Labour Office), continues education and is under 26 years old. After the registration of the portable document S1/E109, the student receives a certificate from the NHF entitling him to full health care services in Poland. The costs of the benefits provided are covered by a foreign health insurance institution.

In case of students and doctoral students who are under 26 years of age and are not subject to any other insurance obligation (they do not have parents, grandparents or legal guardians or their parents, grandparents, legal guardians or spouse are not subject to the health insurance obligation) and to whom the foreign institution would refuse to issue a portable document S1/E109, after recognizing the student's or doctoral student's situation, the university may insure them.

  • In order to apply for health insurance by the university, the student/doctoral student should submit a declaration of not being subject to health insurance obligation under another title.

  • If the above mentioned conditions are not met, it is possible to conclude a voluntary insurance agreement with the National Health Fund.

Students/doctoral students who are not citizens of Poland
  • A student/doctoral student who is not a citizen of a Member State of the European Union or a Member State of the European Free Trade Association (EFTA) and is not subject to compulsory health insurance on the territory of the Republic of Poland and in another Member State of the European Union or a Member State of the European Free Trade Association (EFTA) may take out insurance on a voluntary basis by concluding a contract with the National Health Fund if he resides on the territory of the Republic of Poland.

    The contract is concluded on the basis of a written application and a certificate confirming student status and passport. A student who concludes a contract and pays premiums in a timely manner has the right to a full range of health care services in Poland in facilities that have concluded an agreement for health care services with the NHF. Within 7 days of signing the contract, the student is obliged to register it with the Social Insurance Institution (ZUS ZZA). The contribution for the contract should be paid each month to the account indicated by ZUS, by the 15th day of the following month at the latest. The contract is concluded for an indefinite period. The contract is terminated on the basis of a written notice of termination or after a month of uninterrupted arrears in the payment of premiums or the creation of a title to compulsory health insurance, e.g. conclusion of an employment contract. The document confirming the health insurance coverage is a copy of the contract and a proof of payment of the premium for the last month. The right to health insurance benefits begins on the day of coverage with the insurance specified in the Contract and expires after 30 days from the date of termination of health insurance in the National Health Fund.

  • A student/doctoral student who is not a citizen of a Member State of the European Union or a Member State of the European Free Trade Association (EFTA) and is not subject to compulsory health insurance on the territory of the Republic of Poland and in another Member State of the European Union or a Member State of the European Free Trade Association (EFTA), is recognized as a person of Polish origin within the meaning of the provisions on repatriation on the basis of a decision of the Consul or who has a valid Pole’s Card may be insured voluntarily by concluding a contract with the National Health Fund if he resides on the territory of the Republic of Poland. Health insurance premiums are paid by the University after filling in the ZZA form together with a declaration and submitting the contract with the National Health Fund. The printout together with the declaration and the contract with the National Health Fund should be submitted to the Student Mobility Section of TUL in room 113 of the International Cooperation Centre at ul. Żwirki 36, building A16.

  • A student/doctoral student who is a citizen of a Member State of the European Union or a Member State of the European Free Trade Association (EFTA) is entitled to free health care services provided by healthcare providers who have concluded agreements on the provision of health care services with the National Health Fund on the basis of the European Health Insurance Card or - if he does not have the title to be insured in the country of residence - is declared for insurance by the University on the rules applicable to Polish students.
