Basic information on health insurance for students and doctoral candidates of Lodz University of Technology.

The rules of health insurance are regulated by the provisions of the Act of 27 August 2004 on financing healthcare services from public funds (Journal of Laws No. 164, item 1027, as amended). Students are subject to compulsory health insurance and are entitled to receive free medical care services in facilities which have concluded an agreement for the provision of health care services with the National Health Fund.

1. Students and doctoral candidates of Lodz University of Technology who are citizens of the Republic of Poland

I. Students and doctoral candidates of Lodz University of Technology who are at least 26 and are not subject to compulsory insurance due to other reasons (e.g. employment, business activity, pension, or the status of a family member registered for insurance of the main insured person) are registered for health insurance, at their written request, by the university. Pursuant to Article 2(11) of the Act of 16 November 2022 amending the Act on the professions of physician and dentist and certain other acts, as of 1 January 2023. The University was exempted from the obligation to pay the health insurance contributions financed from the state budget for students and doctoral candidates.


The declaration, together with the relevant Polish Social Security Institution (ZUS) form, must be submitted to the Department of Student Affairs.

The university insurance becomes valid from the day the abovementioned documents are submitted.

The fact of taking up employment or acquiring the right to be insured for another reason must be reported to the university within 7 days (via the ZUS ZWUA form or a statement).

The student is obliged to register their family members for health insurance, provided they are not otherwise entitled to health insurance. The student's entitlement to health care benefits expires 4 months after the student's graduation or removal from the list of students.

II. Students and doctoral candidates of Lodz University of Technology who are under 26 years of age and benefit from their parents' health insurance as family members.

In the case of students/doctoral candidates residing in Poland whose parents reside, work and are subject to health insurance abroad, it is possible for a foreign health insurance institution to issue a portable S1/E109 document which entitles the student/doctoral candidate to receive full health care services in the territory of Poland. The aforementioned document should be submitted for registration at the Provincial Branch of the Polish National Health Fund (NFZ) competent for the place of residence in Poland.

Registration of the S1/E109 portable document with the Polish National Health Fund (NFZ) is possible if the student/doctoral candidate is not entitled to receive insurance in Poland (e.g. due to employment, business activity, pension, or registration with the Employment Office), continues education and is under 26 years of age. After the registration of the S1/E109 portable document, the student receives a certificate from the Polish National Health Fund, entitling them to receive full health care services in Poland. The costs of the benefits provided are covered by the foreign health insurance institution.

Students/doctoral candidates who are under 26 years of age and are not subject to compulsory insurance for a different reason (have no parents, grandparents or legal guardians, or their parents, grandparents, legal guardians or spouse are not subject to compulsory health insurance; do not have a contract of employment; are not self-employed), and who would not be granted the portable S1/E109 document by a foreign institution, can be insured by the University after the student’s/doctoral candidate’s situation has been analysed.

  • In order to be enrolled for health insurance by the university, the student/doctoral candidate should submit a declaration that they are not subject to the obligation of health insurance on any other account.

  • If the abovementioned conditions are not fulfilled, it is possible to enter a voluntary insurance contract with the Polish National Health Fund (NFZ).

2. Students/doctoral candidates who are not citizens of the Republic of Poland

I. A student/doctoral candidate who is not a citizen of a European Union member state, a European Free Trade Association (EFTA) member state or the United Kingdom, and who is not subject to compulsory health insurance in the Republic of Poland and in another European Union member state, a European Free Trade Agreement (EFTA) member state, or in the United Kingdom, may take out voluntary insurance by entering an agreement with the Polish National Health Fund (NFZ), if his/her place of residence is on the territory of the Republic of Poland.

The agreement is concluded on the basis of a written application, a certificate confirming the student's status and a passport. A student who enters the agreement and pays his/her contributions on time, is entitled to the full range of health care services in Poland in facilities that have entered a contract for health care services with the Polish National Health Fund (NFZ). Within 7 days of entering the agreement, the student is obliged to register it with the Polish Social Insurance Institution (ZUS) by submitting the ZUS ZZA form. The contribution must be paid each month to an account specified by the Polish Social Insurance Institution (ZUS), by the 15th of the following month at the latest. The agreement is concluded for an indefinite period. The agreement is terminated on the basis of a written notice or after one month of uninterrupted arrears in the payment of contributions, or the emergence of circumstances entitling the student to compulsory health insurance, e.g. conclusion of an employment contract. A document confirming the health insurance coverage is a copy of the agreement and a proof of contribution payment for the previous month. The right to health insurance benefits begins on the date of coverage specified in the Agreement and expires 30 days after the date of termination of health insurance with the Polish National Health Fund (NFZ).


II. A student/doctoral candidate who is not a citizen of a European Union member state, a European Free Trade Association (EFTA) member state, or the United Kingdom, who is receiving their education in the Republic of Poland, recognized as a person of Polish origin pursuant to the provisions on repatriation, or having a valid Pole’s Card (Karta Polaka), who is not otherwise entitled to compulsory health insurance, is reported for compulsory health insurance by the University after completing the ZZA form with a declaration which must be submitted to the Department of Student Affairs.

Such a person cannot be covered by voluntary insurance.

III. A student/doctoral candidate who is a citizen of a European Union member state, a European Free Trade Agreement (EFTA) member state, or the United Kingdom, is entitled to free health care services provided by companies that have entered contracts on the provision of health care services with the Polish National Health Fund (NFZ) on the basis of the European Health Insurance Card or, if the student is not entitled to insurance in their country of residence, they are reported to be insured by the University, in accordance with the rules applicable to Polish students.

III. A student/doctoral student who is a citizen of a European Union member state, a European Free Trade Agreement (EFTA) member state, or the United Kingdom, is entitled to free health care services provided by companies that have entered contracts on the provision of health care services with the Polish National Health Fund (NFZ) on the basis of the European Health Insurance Card or, if the student is not entitled to insurance in their country of residence, they are reported to be insured by the University, in accordance with the rules applicable to Polish students.

III. A student/doctoral candidate who is a citizen of a European Union member state, a European Free Trade Agreement (EFTA) member state, or the United Kingdom, is entitled to free health care services provided by companies that have entered contracts on the provision of health care services with the Polish National Health Fund (NFZ) on the basis of the European Health Insurance Card or, if the student is not entitled to insurance in their country of residence, they are reported to be insured by the University, in accordance with the rules applicable to Polish students.
